New Patient Registration

Provided your permanent address falls within our practice boundary,(you can check here or above) you have the right to apply for inclusion on our list for medical services. Your details are sent to the Local Health Authority and your medical records will be forwarded to us from your previous GP.

You will be asked to attend for a New Patient Check in order to establish any ongoing medical needs.

We work as a partnership and as such, GP’s do not have individual lists of patients that they see alone, although every patient has a “Named Accountable GP”. You can ask for a particular GP to be listed as your doctor and wherever possible we will try to accommodate this. Furthermore, we will try to ensure you can have continuity of care by providing appointments wherever possible with the GP of your choice. However, there may be circumstances where this is not possible.
